-All initial foot care appointments will include feet assessment,
a professional Golden Age Mobile Spa foot care treatment.
Recommendations for any treatment(s) and suggested scheduling for future appointments will be discussed.
-A $25 fee will be charged to the client, should an appointment be cancelled by the client without 48 hours prior notice. This fee can be added to the client's next scheduled service, at the discretion of Golden Age Mobile Spa.
-Client must answer the medical and personal questionnaire pertaining to their health truthfully. Golden Age Mobile Spa will not be held responsible for any wrong doing due to disclosures, intentional or otherwise, made or not made by persons receiving treatments and services.
-Client consents to pictures that may be taken prior to treatments and/or afterwards if the specialist feels it is necessary for Golden Age Mobile Spa to keep on file. Photos and client information are the property of Golden Age Mobile Spa. They will be used strictly for the professional purposes of examining and assessing the client's ailments, treatments and progress. In some instances, photos and client file documentation can be made available to the client's medical practitioner with in 48 hours of the client's or mandatary's written request. Otherwise all medical and personal information will be kept confidential by Golden Age Mobile Spa.
-Documentation of each service provided to the client includes procedure and treatment(s) received, recommendations made and follow up.
-Should one of our trained professionals feel it necessary or recommend that the client be seen by Podologist, an appointment can be made for the nearest availability at the same cost of $45
-Fee of $50 will be charged should a check be returned for insufficient funds
